How to Set Up a Guest Waiver with QR Code

This guide walks you through downloading a waiver form from the Member Splash Template Library, customizing it for your facility, embedding it on your website, and creating a printable QR code for front desk use.


Step 1: Download the Waiver - Guest Form from Template Library

  1. Log into your dashboard

  2. Navigate to Forms in the left sidebar

  3. Click on MS Form Templates Library at the top of the Forms page

  4. Browse or search for "Waiver - Guest"

  5. Click the Preview icon (eye symbol) to review the form structure

  6. Once confirmed, click Install to add it to your forms list

  7. The form will now appear in your Forms list with the title you assigned during installation

Note: The template includes pre-built fields for guest information and waiver language. You can customize any aspect of the form after installation. A copy of the completed waiver form will go to the club's admin email and a copy will be sent to the email address provided by the guest.


Step 2: Update the Waiver Terms

  1. In your dashboard, go to Forms and locate your newly installed Waiver - Guest form

  2. Hover over the form and click Edit

  3. Update Terms & Conditions / Liability Waiver terms

  4. Update or remove Photo / Video Release terms

  5. Update or remove Health & Safety Acknowledgement terms

  6. Update or remove Guest Responsibilities terms

  7. Review other fields (name, email, phone, emergency contact) and adjust as needed

  8. Click Update to save your changes


Step 3: Embed the Form on a Page

  1. In your dashboard, go to Pages > Add New

  2. Name the page "Guest Waiver" or "Daily Guest Registration"

  3. In the page editor, click the Add Form button

  4. Select the Waiver for Guest(s) form

  5. Add any introductory text above the form if desired (e.g., "Please complete this waiver before your visit")

  6. Set page visibility to Public

  7. Click Publish

  8. Copy the page URL from your browser address bar (e.g., yourdomain.com/guest-waiver)


Step 4: Generate a QR Code

  1. Go to a free QR code generator:

    • QR Code Generator (qr-code-generator.com)

    • QRCode Monkey (qrcode-monkey.com)

    • QR Code Generator (the-qrcode-generator.com)

  2. Paste your waiver page URL

  3. Test the QR code first by scanning with your phone

  4. Customize if desired (add your logo, change colors to match branding)

  5. Download as PNG or PDF in high resolution (at least 300 DPI for printing)


Step 5: Create Front Desk Signage

  1. Open a document in Word, Canva, or your preferred design tool

  2. Insert the QR code image

  3. Add clear instructions:

2. Complete the waiver form
3. Check your email for confirmation
4. Show your confirmation to staff on return visits

4. Print on cardstock or laminate for durability

5. Consider printing multiple copies


Tips for Success

βœ“ Accessibility: Keep a paper version of the waiver available for guests without smartphones
βœ“ Data management: Set up a system to periodically review guest waiver submissions in Gravity Forms entries


Troubleshooting

QR code doesn't scan:

  • Ensure you're using a high-resolution image (300+ DPI)

  • Check that the URL is correct and the page is published

  • Try a different QR code generator

Form not displaying on page:

  • Check that the form is set to Active in Forms list

Guests not receiving email confirmation:

  • Check spam folders

  • Verify email notifications are enabled in Form Settings

  • Test with your own email address first
